Who is SeniorCare HHA?
SeniorCare HHA is a New York State Managed Long Term Care Plan designed for adults living with long-term disabilities. Our goal is to help our members live safely and independently in the comfort of their own home. At SeniorCare HHA, you will have a dedicated team of nurses and social workers who will ensure that your care and services help you live as independently as possible.
Our Services
SeniorCare HHA provide HHA/PCA, CDPAP, Private Duty Nursing services
that can be tailored to meet your needs:
- Meal Preparation
- Errands and Grocery Shopping
- Assistance with Ambulation
- Companionship
- Medication Monitoring, Reminders, and Pours
- Assistance with Exercise Regimens/Range of Motion
- Assistance Upon Discharge from Hospital
- Nursing Visits from a Registered Nurse
- Alzheimer’s & Dementia Care
- Insulin Injections, Wound Care Treatment, and more
- Personal Care
- Light Housekeeping
Are you eligible?
To enroll in SeniorCare HHA, you must be:
- 18 years old or older
- A resident of Manhattan, Queens, Bronx, Brooklyn, or Staten Island; or a resident of Nassau, Suffolk or Westchester Counties*
- Eligible for Medicaid
- Expected to require the long-term care services offered by the program for at least 120 days
* We provide CDPAP service for all Counties without exceptions and HHA/PCA for all except Suffolk and Westchester Counties.
